Virtual Appointment

• Even with this virtual appointment, some in-person appointments will still be needed.
• Note that your request will be sent to Oxmoor Valley Ortho via email.
• Oxmoor Valley Ortho is not responsible for any data charges that may occur using the virtual evaluation.

You will need 2 spoons to hold your cheeks out of the way. It’s also recommended that you have a someone help you take photos. Be sure to use flash on your camera to help light your teeth and mouth. The next few images are examples of how to take each photo. You will be able to upload all of your photos at the end of the form.

Once your submission is received, your information will be reviewed. If you're a new patient and this is a Virtual Consultation submission, we will contact you as soon as possible to share your results and recommend options. If you're an existing patient and this is a Virtual Appointment submission, we will contact you only if any additional info is needed!

Note - You should receive an email confirmation once you complete this virtual appointment. If you do not get an email confirmation, please contact the office to ensure your info was received.
